Home Instead is an agency that provides in-home care to seniors in west Washington County, Yamhill County, and Columbia County. Currently, our client care team is working with 230 seniors and various community agencies. This position will be full-time, and includes a significant amount of time in the field meeting with clients and assessing their care needs.
Care Manager Job Description
Overview:
· This position requires the performance of office and field work directly related to the general business operations of Compass Enterprise, Inc. dba Home Instead.
· This position requires the exercise of discretion and independent judgment in regard to the significant matters of developing relationships with our clients and their representatives and of maintaining ongoing billable services for our senior clients.
Experience with In-Home Care or Resident Care Coordination preferred.
Objectives:
· Meet with prospective clients to discover their needs, sign them up for service, and establish a billing relationship
· Ensure satisfaction, safety, and quality care through regular phone contact with clients and representatives as well as live visits in their homes.
· Uncover opportunities to protect and expand service hours with the aim of improving the overall value we provide to clients.
Reporting Relationship:
· Report to the Director of Client Care on a daily basis. Use the Director of Client Care as a first point of contact for most questions pertaining to client responsibilities. The Owner, Managing Director, and other staff are always available to you as well.
· Provide daily updates to the Director of Client Care, via electronic means or live conversations, regarding important client issues. Initiate ongoing and constant contact.
Primary Duties:
· Reflect the values of Compass Enterprise Inc. dba Home Instead
· Be a point of contact in the office for clients and family members to facilitate problem resolution for client care issues.
· Conduct Care Consultations in the field
· Keep sufficient documentation of all relevant phone and in-person contacts with clients, client representatives, CAREGivers and others.
· Responsible for ensuring all quality assurance home visits with new clients two weeks after start of service and every 90 days are scheduled and completed by the due date. Ensure proper maintenance of client journals. Collect service records for office file and keep journal stocked with blank forms.
· Maintain all active prospects and client records in Clearcare. Update and inactivate prospect and client files as needed.
· Take phone inquiries from prospective clients, set up service calls, record client inquiries in Clearcare to enable reporting conversion, closing and referral sources.
· Know about long term care insurance and how it works to be in a position to advise clients on this topic during care consultations.
· Responsible for maintaining client files; scanning into the main shared folder and ensuring the hard copies are in the client file.
· 90- Day audit of client files when Home Visit is conducted.
· Manage the service relationship with the Washington/Yamhill/Columbia County Family Caregiver Support Program/Medicaid/VA including receiving referrals, documenting our responses to referrals, setting up service calls, and following up county contracts.
· Gain mastery of the following technical systems/processes/resources: Google, Clear Care, Google Calendars, Integra Phone System, MapPoint, and Adobe Acrobat for Document Scanning, Email to Fax, Instant Messaging, Text Messaging, Google Drive, HelloFax, Postage Machine, The Franchise Center, and more.
Secondary Duties:
· Conduct client /CAREGiver introductions
· Requires intimate knowledge of all care consultation paperwork: Client Disclosure Statement, Client Assessment, Service Plan, Service Agreement, Medication Services Agreement, and Direct Payment Authorization.
· Assist with on-call duties as needed
· Assist as a backup for weekend care consultations in urgent cases. These care consultations will be scheduled at the discretion of the Director of Client Care.
